Due to AZ Campaign laws please keep the below guidelines in mind whether you are donating online or downloading the pdf file and mailing it along with a check to the below address.
Guidelines:
1.
Any adult (individual)
in
the
United
States
can
donate
to Rick’s
campaign. This
does exclude
businesses.
2.
Personal
Checks
are
permissible,
however
no
business checks
may
be
accepted.
3.
Name
of
the
contributor
on
this
form
must
match
the
name
on the
check. If
a
married couple
both
want
to
contribute
and
use
one
check,
the
check
must
have
both names
printed
on the
check
and
both
parties
must
sign
the
check.
4. Make
checks

5. Donations of
any
amount
are
appreciated. Maximum allowed
per
person
is
$424.00 or $848.00 per couple.
